What that means for a provider

Because the loser effectively bears the entity fee, and providers prevail in roughly 85% of decided disputes, the expected fee cost per dispute is low.
The administrative fee cut from $115 to $15 removed most of the fixed cost of filing lower-dollar claims.
Batching spreads the entity fee across many claims — the main reason emergency and radiology groups can dispute even modest underpayments profitably.
The real cost is staff time and missed deadlines, not fees.
