What's included

Eligibility audit

We review your out-of-network remittances, apply the federal and state eligibility rules, and produce a claim-level list of what can be disputed, what should be batched, and what isn't worth filing. Ineligible disputes were 42% of challenges in late 2025 — filing them wastes fees and clogs your queue.

Open negotiation

Every eligible claim gets a compliant open negotiation notice within the 30-day window. Where payers negotiate in good faith, we settle at a number supported by determination data and avoid IDR fees entirely.

IDR initiation and batching

Disputes are initiated in the 4-business-day window after open negotiation ends, batched under current rules, and filed through the federal portal with the required documentation. We manage IDR entity selection and objections, eligibility responses, and administrative and entity fees.

Offer strategy and written submission

The arbiter picks one of two offers. We build yours from actual award data for the same codes, specialty, and market, and support it with the factors arbiters weight: provider training and experience, case complexity, market share, prior contracted rates, and good-faith negotiation.

Award follow-through

Payers owe payment within 30 days of a determination and frequently miss it. We track every award to cash, escalate late payments, and file CMS complaints when needed.

Reporting

Monthly reporting by payer, specialty, and code: filed, settled, decided, won, and collected — with award-to-QPA ratios so you can see exactly what IDR is adding.
