What Pivotal Health is

Pivotal Health is a technology-led entrant in the No Surprises Act dispute space, founded in the mid-2020s and backed by institutional venture capital. Its go-to-market leans on revenue cycle management partnerships and larger physician organizations, and it operates on a success-fee model tied to recovery.

We're not going to characterize their win rates or pricing — ask them directly and get it in writing. What we can compare is the shape of the two offerings.

Where the models differ

  • Who does the work. Platform-led models expect your team or your RCM partner to feed the process. We file end to end: eligibility screening, open negotiation notices, initiation, IDR entity selection, offer and rationale, post-award collection.
  • Who they're built for. Enterprise-oriented vendors optimize for large, standardized books. Independent groups with a few hundred disputes a quarter tend to fall below that threshold and get commodity service.
  • What the fee applies to. Ask any vendor whether the percentage applies to the total award or only to the amount recovered above what the plan already paid. The difference is large.
  • Data ownership. Ask whether you keep your dispute history and determination outcomes if you leave. That history is leverage in your next contracting cycle.
A fair reason to pick them over us: if you're a large enterprise already integrated with an RCM partner in their network and you want a platform you can operate yourself, that's a real fit. We're a better fit for independent hospital-based groups that want the work done, not tooled.

What to ask both of us

  1. What percentage of filed disputes come back ineligible?
  2. How are offers constructed, and from what data?
  3. Average items per batch, and how batches are assembled under the 2026 rules?
  4. Who chases the 30-calendar-day payment deadline after an award?
  5. What does the fee apply to, and what happens to declined claims?
